Volunteer Checklist

Our Lady of Calvary School is so grateful for its many community and parent volunteers. We depend on volunteers and always need help with our many events at school.

Please fill out form below and send into school or email relevant info below to j.costello@olcalvaryschool.org.

Please remember that all volunteers and chaperones need to have their PA State Criminal Background check, and get a Child Abuse Clearance.  Both are free and can be completed online.

Websites for both clearances:  http://epatch.state.pa.us (criminal background check) and https://www.compass.state.pa.us/cwis/public/home (child abuse clearance)

The forms for the disclosure statement and mandated reporting can be found at the bottom of the main page under FORMS.  Please download, print out the forms, sign the forms, and send in to school addressed to Mrs. Costello.

Please contact Vice-Principal Jeanne Costello at j.costello@olcalvaryschool.org with any questions.

If you are a parent who is interested in chaperoning a class trip, your clearances have to be in school one week prior to the trip or you can not attend.

Thank you so much for your continued support.
